What is The Transitions Center?
The Transitions Center of Hospice Care Corporation is a community-based program that provides services throughout North Central West Virginia. The services of the Center are available on-site, in the home, or in one of our seven offices. We offer support in many forms to anyone who has experienced a loss due to death or other life circumstance.
Our services include a variety of educational and support groups, individual counseling for children and adults, resource and referral services, school and workplace programs, a children’s grief camp, and crisis response. Our team of grief and loss professionals are here to help you or your organization.
The Transitions Center also provides education, training, consultation and resources to those who work with the bereaved, such as school personnel, employers, health care providers, funeral home staff, religious leaders and the community.

The Transitions Center responds to community crisis events in the school, workplace, and other community sites.
Children’s Bereavement Camp – Camp Nabe (Nah’ bee)
Camp Nabe (Nah’ bee) is a bereavement camp designed for children 6-14 who have experienced the death of a loved one. Since 1996, Hospice Care Corporation has held Camp Nabe to support children and teens in West Virginia who have lost a loved one and provides the child an opportunity to spend a day with other children who have also experienced loss. This one day camp is free of charge and is open to any child age 6 – 14.
Hospice Care has added grief workshops for the surviving adults in the child’s life. These sessions will run at the same time as the children’s camp. The purpose is that each family member is individually strengthened and supported so that the entire family can move forward together on their journey of grief to a place of new normal.
